Credit Card Authorization
Please fill out the secure Authorization Form to allow TLS to bill your credit card for your transportation rental.
After submitting, a TLS representative will call you to obtain the credit card number.
Parental Consent
Please fill out the secure Parental Consent Form to provide TLS with the name of the responsible party for the transportation rental of minor passengers.
